Terms and Conditions for PRIVATE BAND TREATMENTS

• Only 1 Course of Treatment per patient over a 6 month period.
• Band 2 includes BAND 1 + 1 treatment items e.g. 1 extraction or 1 filling
• Band 2 excludes root canal treatments/Surgical Extractions
• Band 3 includes Band 2 + 1 crown or 1 denture (upper or lower)
• Synergy reserve the right not to accept any course of treatment for any private banded course of treatment.
